Forward Sagging of the Shoulder – Causes, Diagnosis and Treatment in Dubai at Emirates Hospitals Group

Overview

Forward sagging of the shoulder, also known as shoulder protraction or rounded shoulders, occurs when the shoulders slouch forward instead of aligning properly with the spine. It can lead to muscle imbalances, pain, and posture-related issues.

Causes

  1. Poor Posture – Prolonged sitting, hunching over desks, or excessive screen time can weaken back muscles and cause forward shoulder positioning.
  2. Muscle Imbalance – Weak upper back muscles (trapezius, rhomboids) and tight chest muscles (pectorals) contribute to poor shoulder alignment.
  3. Injury or Overuse – Repetitive overhead movements or past shoulder injuries may lead to muscle tightness and imbalance.
  4. Osteoporosis – Weakened bones due to osteoporosis may cause poor posture and forward shoulder sagging.

Diagnosis

  1. Physical Examination – A doctor or physiotherapist assesses shoulder alignment, muscle strength, and flexibility.
  2. Postural Assessment – Evaluating posture while standing and sitting helps identify muscle imbalances.
  3. Imaging Tests – X-rays or MRIs may be needed if underlying conditions like arthritis or fractures are suspected.

Treatment Options

  • Postural Correction Exercises – Strengthening the upper back and stretching the chest muscles can help realign the shoulders.
  • Physical Therapy – Guided exercises and manual therapy improve shoulder positioning and mobility.
  • Ergonomic Adjustments – Using an ergonomic chair, maintaining proper desk height, and taking breaks from prolonged sitting can prevent worsening of posture.
  • Bracing or Taping – Shoulder braces or kinesiology tape may provide support and improve posture awareness.
  • Medical Treatment – In cases linked to osteoporosis or joint conditions, medications and bone-strengthening therapies may be recommended.

Early intervention can prevent long-term complications and improve overall posture and musculoskeletal health.
