Total Knee replacement is a surgical procedure to resurface a knee damaged by arthritis. Metal and plastic parts are used to cap the ends of the bones that form the knee joint, along with the kneecap. This surgery may be considered for someone who has severe arthritis.

Orthopedic Pediatric Surgery

Pediatric (Child) Orthopedic Clinic deals with muscle and skeleton illnesses such as hip dislocation, hip dysplasia, pes equino varus (crooked foot), spina bifida (open spinal cord), pes planus (flat foot) that appear in childhood, due to congenital or acquired neurological, infectious, tumoral, traumatic and other reasons It is a sub-department of the Department of Orthopedics and Traumatology, which deals with the diagnosis and treatment of system diseases. Child orthopedics mainly covers the childhood and young adult patient group. Under this heading, the pediatric lower limb is subdivided into the pediatric upper limb and the spine.

The main purpose is to provide the patient to return to normal life without sequelae in the long term by performing diagnosis and treatment in a short time. Many of the orthopedic problems that occur in adulthood are caused by pediatric orthopedic diseases that have not been diagnosed, treated, or properly followed up in childhood. For this reason, pediatric orthopedics has an important place in reducing adult orthopedic problems. Pediatric orthopedics is one of the most basic and important areas of orthopedic surgery and occupies an important place in public health.

Congenital hip dislocation: It is a condition that occurs due to insufficient development of the playful area that forms the hip joint. The incidence is higher in girls, twins, babies with similar diseases in their families and swaddled babies. Signs of hip dislocation are the difference in length between the legs in the early period, the knuckles in the legs are asymmetrical, and limping is observed in children who have started walking. Babies should be checked periodically for this condition. Sometimes ultrasound and x-rays are required in addition to the examination. Its diagnosis in the early period facilitates the treatment and provides a smooth recovery. Delayed hip dislocations may need to be treated with cast or surgical methods. In neglected babies, severe conditions may occur, resulting in shortness of legs and limping.

Cerebral palsy: It is the condition of damage in the brain due to the lack of oxygen in the baby during childbirth. Cerebral palsy affects movement, balance, and posture. Symptoms of cerebral palsy can range from mild to severe and worsen as the child grows. With treatment, significant improvement can be achieved in most children.

Perthes Disease: Perthes disease, which is seen only in children, is a condition that occurs with the impaired blood supply of the femoral head epiphysis. Bone part, whose nutrition is impaired, loses its vitality; this condition is called avascular necrosis. The rare Perthes disease affects the hip joint of children between the ages of five and ten, causing pain and limping.

Congenital toe: It is the most common congenital foot deformity. The affected foot appears with the wrist turned inward. It is seen in 1-2 births in 1000. It is 2 times more common in baby boys. Mechanical pressure in the womb, foot cartilage development problems, genetic conditions, and nerve lesions cause congenital crooked feet.

Scoliosis (Spinal Curvature): Scoliosis (spinal curvature) seen in children before the age of 10 may bring along many problems such as breathing problems. Scoliosis that starts at a young age can progress rapidly due to physical development. Careful parents can easily find out if their child has scoliosis, or they can resolve the problem at an early age. Symptoms:

  • Increased pubescence, color changes and dimples in the back
  • Balance problems
  • Back protrusions when bending forward
  • Shoulder height is not equal
  • Side stance on waist or hips

Disproportionate posture on the legs relative to the trunk

Shoulder Arthroscopy

The shoulder is a very moving joint and therefore often suffers injuries. As a result of injury, complaints such as pain, tenderness, weakness, laxity, and restriction of movement may occur. After a good examination and necessary diagnostic methods, shoulder arthroscopy may be required to understand and treat exactly what the problem is. Arthroscopy is a very fast and easy method.

Arthroscopy:

Monitorizing of the joint inside

By making small cuts with the arthroscopy method, you can see the inside of your shoulder joint and make an operation here. A thin, long and illuminated device called an arthroscope is used for this procedure. During the surgery, the arthroscope sends live video images taken from the joint to the monitor. Thanks to these images, the problem on your shoulder can be identified and treated. Since the instruments used in the arthroscopy method are very thin and small, very small cuts are made on the skin, so the recovery period lasts much shorter than open surgery, which is an important consideration for the patient’s comfort.

Risks and possible complications of arthroscopy:

  • Stiffness and constant pain in your shoulder
  • Bleeding or blood clotting
  • Infection
  • Damage to nerves or vessels

When is Shoulder Arthroscopy Recommended?

If the pain in your shoulder does not go away with rest, physical therapy, medication, or local injection applications, your doctor may recommend shoulder arthroscopy.

The most common arthroscopic repairs are;

  • Rotator cuff tears
  • Shaving bone protrusions
  • Repair of labrum tears
  • Tendon repair
  • Removal of inflamed tissues and free cartilage in the joint
  • Repetitive shoulder tip repair
  • Cyst excision
  • Surgical Procedure

The clearest image is tried to be obtained by injecting liquid on the shoulder of the patient who is placed in the appropriate position. The shoulder joint is entered with a camera called atroscope, and the problems of the shoulder joint are evaluated from the monitor. After the problem is clearly identified, repair is done using small hand tools with a separate incision to repair it.

Treatment of shoulder problems with Arthroscopy

Impingement

Some arm movements become painful and problematic as a result of long-term use of the arm over the head. Arthroscopic treatment can be applied to reduce pressure and move the arm more comfortably. If bursa is swollen and thickened, it can be removed during surgery and acromion protrusion can be shaved if any. Coracoacromial ligament can also be cut.

Rotator Cuff Tear

The rotator cuff can tear due to sudden injury or excessive use. This can lead to pain, weakness in the arm, and loss of normal shoulder movements. During surgery, torn rotator cuff tendons can be shortened to remove the torn region. After that, the beams are attached to the arm bone again. For this, stitches, anchors or surgical screws are used.

Labrum Tear

The labrum can tear off the edge of the glenoid slot. In this case, a joint may occur or feel as if it is dislodged. It may even be the dislocation of the shoulder. It is possible to repair the torn labia by attaching it to the glenoid again.

Arthritis and Loose Objects

The damage caused by cartilage in the joint as a result of old age, overuse, injury or rheumatic diseases is called arthritis. Some loose objects such as bone or cartilage fragments or bone protrusions may appear inside the joint due to wear. Bone protrusions are cleaned during surgery. Rough areas of the joint are smoothed. Loose objects, if any, are also cleaned from the joint.

The most common Sports Injuries are;

Muscle – tendon injuries

Tears and tendon ruptures in the muscle fibers (Achilles tendon ruptures, shoulder rotator cuff tears, quadriceps tendon ruptures, tendon ruptures in the groin, hamstring muscle ruptures)

Ligament and meniscus injuries

Strain, stretching and breaks in the ligaments as a result of sprains and twists in the joints (ankle ligament injuries, internal lateral ligament, external lateral ligament, anterior and posterior ligament injuries, internal or external meniscus injuries)

Cartilage injuries

Knee, elbow, shoulder and hip joints are cartilage injuries in order of frequency.

It is possible to take some precautions to prevent sports injuries. It is the first precaution to obtain information about sports to be done and to obtain appropriate materials. Doing warm-up stretching exercises for about half an hour before starting the sport will add flexibility to the body. Even if there is no previously known discomfort, if you experience extreme fatigue, weakness, heart palpitations, and dizziness during the sport, the sport must be terminated.

The most commonly used method in the treatment of sports injury is arthroscopy. In arthroscopy (closed surgery), in order to examine the problematic joint, an incision smaller than 1 centimeter is thrown into the area and the joint is examined with a pen-sized system and a tiny camera. A liquid is filled in the area for a clear view and swelling of the joint is ensured. This fluid is discharged again after imaging. Arthroscopy is a very preferred method because it provides healing faster. The patient starts physical therapy exercises shortly after the operation and accelerates the healing process. Arthroscopic surgery is widely used in the treatment of diseases involving the knee joint, shoulder, hip, ankle, elbow, and wrist joints.
